A 64-year-old man with LUTS, PSA of 3.0 ng/mL, and a DRE confirming moderate prostatic enlargement was started on an alpha-blocker. He returns to your office 1 month later stating that he “doesn't think it has helped much.”
At this point you:
a. Continue with the alpha-blocker for an additional 2 months to ensure an adequate trial of therapy
b. Continue with the alpha-blocker and add a 5-alpha reductase inhibitor
c. Add an antimuscarinic agent if his symptoms are primarily storage related and he empties his bladder well
d. Perform urodynamic studies
e. Recommend minimally invasive therapy
f. Recommend cystoscopy and TURP (laser or otherwise)
